This section contains applications for many of the loan products offered by County Bank. You are invited to consult with a County Bank Loan Officer or Personal Banker prior to completing the application or form to assure that you are completing the appropriate form and to discuss any additional information that may be required. Upon completion of the application, please bring it to a County Bank office or mail it to your Loan Officer or Personal Banker. County Bank’s website is not a secure site and we recommend that you DO NOT e-mail applications or forms that contain personal account information.

Loan Applications
This application can be used for most consumer loans at County Bank. This includes auto loans, personal loans, loans for recreational vehicles and equipment, and any other personal needs.
Open End Credit Line/Safety Check Application
This application is to be used to apply for approval for County Bank’s Safety Check Overdraft Line. This product offers you the ability to have an automatic line of credit for "overdraft protection" or to "write your own loan" up to your pre-approved personal credit limit.
Uniform Residential Loan Application (URLA Form 65)
This application is the government-approved form to be used in applying for residential real estate mortgages. During the application process potential borrowers will also be asked to provide the last 3 years tax returns, a current month pay stub and the last 2 months of bank statements. The application is accompanied by 3 disclosures that the borrower should review and sign as part of the application process. They are:
Universal Credit Application (Consumer Residential Real Estate)
This application is to be completed and submitted as part of the request for a Home Equity Line of Credit (HELOC). The application is accompanied by the Home Equity Line of Credit Disclosure that should be reviewed by the applicant prior to submitting the application. (Note: This application is in 8 1/2x14 “legal” sized format.)

Electronic Account Statement and Check Image Delivery Enrollment Agreement
Use this form to sign up to receive eStatements. County Bank's eStatements are sent monthly to your computer in a password-protected file and include images of your deposit and check activity for the month. A great way to "go green"!
We can make it easy, convenient and hassle-free to make the smart switch to County Bank. Our Switch Kit contains all the forms you'll need to notify your previous bank as well as vendors and direct depositors of your move to County Bank. Simply fill the forms in on your computer and mail them. A County Bank Personal Bank can assist you if you have questions. The first step in talking with a loan officer about an ag loan is the preparation of a balance sheet and financial statement showing the assets and liabilities of your agricultural operation including your farmland, buildings, livestock, crops, machinery and equipment, vehicles and other parts of your financial picture. If you do not have a financial statement now, this form can help you prepare the information you’ll want to discuss with your County Bank loan officer.
